New Patients

We ask that new patients arrive 15 minutes prior to your appointment time to complete the intake forms. To start this process earlier, please fill out the appropriate forms for your appointment. Additional forms may be provided at the appointment. 

Initial appointments start with filling out the intake forms. During your appointment, the therapist will go through your health history and an assessment will be done to determine the exact nature of the injury. Once the therapist has an understanding of your injury, treatment will be done during this initial appointment. Lastly, the therapist will discuss their suggested treatment plan and will work with the reception team to ensure you are taken care of.

About Insurance

The Adelaide Health Clinic therapists are all fully licensed and treatments can be covered by most insurance plans. Prior to the appointment, please confirm with your insurance provider that the service will be covered by them and whether a medical referral is required to claim the appointment.

We do not bill insurance directly – we will provide you with a printed or emailed copy of your receipt.

What should I do when I arrive for my first appointment?

We ask that all patients arriving at the Clinic check in with reception. If it is your first visit at the Clinic, there may be an intake form for you to complete.

What is your cancellation/reschedule policy?

We have a 24 hour cancellation policy. If you need to cancel or change an appointment, you must do so with 24 hours notice to avoid any late cancellation or missed appointment fees.

What do I do if I need to cancel or reschedule my appointment?

You may either phone or email the Clinic to cancel or reschedule appointments. Please note we require 24 hours notice to avoid late cancellation or missed appointment fees.
