Our Clinic Director, Dr. David Lee, is a graduate of National University of Health Sciences in Chicago, one of the nation’s premier institutions in Health Sciences education. In addition to his professional degree, Dr. Lee earned his Bachelor degrees in Science (University of Waterloo) and Kinesiology (York University) providing him with an impressive knowledge base that spans from the basic physiological processes in living organisms to human anatomy and the biomechanics of movement.
Dr. Lee is a passionate student of biomechanics, the study of human movement. He is fully certified in the patented specialized soft tissue technique, Active Release Technique (ART) that is internationally renowned among professional and Olympic athletes. Dr. Lee’s emphasis on the first-line treatment of muscle and nerve dysfunctions for common and stubborn clinical cases has gained him an impressive list of clientele and referral system from medical colleagues throughout the city. Dr. Lee is a firm believer in pain prevention through physical fitness, and brings his experience as a personal trainer of more than 10 years to promote an injury free and active lifestyle.
Dr.Lee is also a passionate student of Leadership, including leadership in business. Dr Lee has attended numerous leadership conferences to learn from top business and political leaders such as Jim Collins (author of Good to Great), General Colin Powell (former US Secretary of State), John Maxwell (author of several leadership books including The 21 Indispensable Qualities of a Leader: Becoming the Person Others Will Want to Follow), Colleen Barrett (President of SouthWest Airlines), Bono (Musician and Aids Activist), and many more. Dr. Lee tries to implement a strong culture of leadership and excellence at the Adelaide Health Clinic. Dr Lee is asked to provide various seminars on ergonomic and injury prevention topics at several companies in the financial district.
Dr Lee is a sports enthusiast and enjoys playing several sports including football, baseball and snowboarding. Dr. Lee has played football with York University, and currently plays Semi-Professional football in the Northern Football Conference with the Milton Marauders. |

Our family physician, Dr. Eddy Jin, earned his Doctor of Medicine degree from University of British Columbia and successfully completed his Medical Residencies in Family Medicine (University of Calgary) and Emergency Medicine (University of Alberta). He also holds a Bachelor of Science specialist degree in Pharmacology and Therapeutics from the University of British Columbia. Dr. Jin holds numerous professional affiliations that include: Canadian Association of Emergency Physicians, College of Family Physicians of Canada, Ontario Medical Association, Canadian Medical Protective Association and the College of Physicians and Surgeons of Ontario.
In addition to Dr. Jin’s practical contribution to healthcare, he has contributed to the wider medical academic community by publishing his research titled “CT Scanning for Minor Head Injuries” in the Canadian Journal of Emergency Medicine. Dr. Jin is currently a full-time Emergency Physician at North York General Hospital and is a part-time addition to the impressive staff at the Adelaide Health Clinic. |

| Our family physician, Dr. Yasmin Ladak, earned her Doctor of Medicine degree from McMaster University and successfully completed her Family Medicine Residency program (McMaster University) as well as her Anesthesia Fellowship Program (University of Toronto). She also holds an Honors Bachelor of Science degree in Psychology from McMaster University. Dr. Ladak holds numerous professional affiliations that include: College of Family Physicians of Canada, College of Physicians and Surgeons of Ontario, Canadian Anesthesiologists’ Society and the Canadian Medical Association.
In addition to Dr. Ladak’s practical contribution to healthcare as a family physician, ER physician and Anesthetist, she has contributed her time and expertise as a volunteer to numerous community services that include: member of the Disaster Management Research Team for Focus Canada, member of the McMaster Student Health Advisory Board, Health Columnist for the McMaster University Newspaper, and volunteer for the Child Life Program (Bloorview Children’s Hospital) and Crisis Help-Line for abused women (McMaster University Safer Space Program), just to name a few.
Dr. Ladak has also contributed to the medical research community through her research endeavors resulting in the following scientific presentations and papers, “Factors that affect anesthesia and the perception of pain among individuals undergoing phacoemulsification cataract extraction with intraocular lens implantation” (St. Michael’s Hospital, University of Toronto), “The influence of mood on quality of life, treatment satisfaction and perception of care received in an outpatient population” (St Joseph’s Healthcare, McMaster University), and “Age-related changes in the 25-hydroxyvitamin D versus parathyroid hormone relationship suggest a different reason why older adults require more vitamin D” (Journal of Clinical Endrocrinology and Metabolism, 88:185-191, 2003), just to name a few.

DR. MARYSE MAZEROLLE, BSc, MD, CCFP

| Our family physician, Dr. Maryse Mazerolle, earned her Doctor of Medicine degree from McMaster University and successfully completed her Family and Community Medicine Residency program at the University of Toronto. She also holds an Honors Bachelor of Science degree with high distinction in Anthropology from the University of Toronto. Dr. Mazerolle holds numerous professional affiliations that include: Ontario Medical Association, College of Family Physicians of Canada, College of Physicians and Surgeons of Ontario, Canadian Medical Protective Association, Canadian Medical Association and the Ontario Medical Association’s political Member Action.
Dr. Mazerolle has contributed her time and expertise as a volunteer to numerous community services that include: volunteer with developmentally delayed individuals with Community on Campus, volunteer at the Peel HIV Aids Network in the Needle Exchange Program, McMaster Medical School Admissions, participated in MAC SAFES in teaching safer sex education to high school students, directed focus groups with high school student to discuss mental health issues, and volunteered with the University of Toronto Family Medicine Mentorship Program.
Dr. Mazerolle is currently a full time family physician at the Taunton Health Clinic in Oshawa, and is an associate staff member at Toronto Western hospital where she supervises residents going through the Family and Community Medicine Residency Program. SUZANNE HELFENSTEIN, BSc, PT

| Our Physiotherapist, Suzanne Helfenstein, is a graduate of the University of Western Ontario B.Sc.P.T. program (cum laude). One of her early job placements was at the Fowler Kennedy Sports Injuries Clinic, in London, Ontario, but for the past 20 years, she has managed her own busy private practice, also in London. Suzanne is a valuable addition to the impressive staff at the Adelaide Health Clinic. She very much enjoys the challenges of receiving referrals from medical specialists for the assessment and treatment their “difficult” patients, that is, those who have not been successful with any other therapy or treatment. Her interest and expertise are in the treatment of back and neck pain, overuse and sports injuries, and dysfunctions, weaknesses, and imbalances due to improper ergonomics and/or biomechanics. She also possesses a strong interest in dealing with the ergonomic and physical challenges introduced by our technology savvy business community.
Throughout her 20 years of practice, Suzanne has developed an impressive and loyal clientele which include elite athletes from the Canadian National Rowing Team, 2 National Heptathletes, and varsity cross-country runners, as well as competitive athletes in hockey, running, squash, tennis, golf, cycling, baseball, volleyball, swimming, soccer, lacrosse and power lifting. Suzanne herself, also enjoys participating in competitive sports such as squash, running, triathlons, and windsurfing and has served as a trainer for competitive hockey teams and a developmental level soccer team.
Suzanne encompasses a pro-active attitude towards getting her patients out of pain, both in work and play. She is passionate about providing the education and tools necessary for her patients to prevent and/or manage their pain as independently as possible. |

Our Physiotherapist, Mark McFadden, is a graduate of the McGill University B.Sc.P.T. program.
Mark’s professional affiliations include: Canadian Physiotherapy Association and Orthopaedic Division and the Ontario College of Physiotherapy. In addition to his professional affiliations, Mark’s credentials include: National Coaching Certificate Level 1, Lumbar Spine Part A of The Robin McKenzie Institute of Canada (Medical Diagnosis and Treatment), Level II (Lower) and Level II (E2V2) of Manual and Manipulative Physiotherapy Program.
Mark has over 10 years of experience as a physiotherapist at the McGill Sport Medicine Clinic, Montreal Children’s Hospital and on various sports teams which incude: Club de Volleyball Boreal, C.E.G.E.P. Bois-de-Bologne AA Volleyball and Les Grand Ballet Canadien de Montreal. Mark has also developed and instructed courses for the C.E.G.E.P. St. Felician Rehab Monitor Training Program, and provided service development for the Cree Board of Health and Services of James Bay. Mark was also successfully involved in the strategic project and planning management for physiotherapy event coverage at the 2006 World Outgames in Montreal (2006). In addition to his impressive practical experience, Mark has taught various courses on the Musculoskeletal System to medical professionals, advanced topics in Orthopaedic Rehabilition at the McGill School of Physiotherapy and served as clinical supervisor for student clinical placement.
Mark is a general sports enthusiast and his interests include: competitive indoor and beach volleyball, tennis, badminton, running, downhill and cross-country skiing, in-line speed skating and swimming. Our registered dietitian, Jean LaMantia, earned her Bachelors degree in Applied Human Nutrition from University of Guelph and completed a clinical dietetic internship at the Toronto Western Hospital. Jean is certified as a Diabetes Educator, and received a certificate of Training in Adult Weight Management from the American Dietetic Association. Jean holds numerous professional affiliations that include: The College of Dietitians of Ontario, Canadian Celiac Association and is the current Chairperson of the Consulting Dietitians Network of Dietitians of Canada.
Jean has provided her expertise in developing personalized nutrition plans for her clients in clinics, hospitals, outpatient diabetes centers, and in-home care facilities. As a registered dietitian, Jean provides counseling on healthy eating, weight loss/gain, cholesterol control, sports nutrition, diabetes, hypoglycemia, osteoporosis treatment and prevention, reducing cancer risk, food allergies, irritable bowel syndrome, high blood pressure, prenatal, pregnancy and vegetarianism.
Jean is a firm believer in educating the public on the benefits of a healthy diet and her approach is holistic, including nutrition as well as exercise and eating behaviors. Our Registered Dietitian, Mary Bamford, earned her Bachelors degree in Clinical Nutrition from the University of Western Ontario and completed a Clinical Dietetic Internship at the University of Alberta Hospitals. She also completed her Masters Degree with Dean’s honours in Business at McMaster University in Hamilton. Mary enjoys working at The Club at White Oaks in Niagara-on-the-Lake and Niagara College in addition to The Adelaide Health Clinic.
Mary provides individualized nutrition assessment, advice and education to help people prevent and treat diseases and maximize athletic performance. She also offers a wide range of group seminars, cooking demonstrations and taste testings. She brings fun and pleasure to food and nutrition.

| Dr. Jumy Kim earned her Doctor of Traditional Chinese Medicine from the Toronto School of Traditional Chinese Medicine. She is also fully certified in acupuncture, herbal medicine and tuina massage. Dr. Kim adds to the impressive academic credentials of the Adelaide Health Clinic staff with a Bachelors degree specializing in Anatomical Sciences (McGill University), a Masters degree in East Asian Studies (McGill University), and is currently a Ph.D. Candidate in the Department of Asian Studies at the University of British Columbia.
Dr. Jumy Kim practices full-time at Dufferin Steeles Acupuncture Centre and is currently a part-time addition to the impressive staff at the Adelaide Health Clinic. Dr. Kim educates the public on the benefits of Traditional Chinese Medicine and Acupuncture through seminars and is a strong advocate of clean-needle techniques. Regan joined the Adelaide Club to become part of a multi-disciplinary team that would have the resources to achieve the goals of each client. Her belief in continuing education has taken her through such courses as Cranial Sacral Therapy and has now led her to the Canadian College of Osteopathy. Here she will continue her pursuit in the field of alternative medicine and will receive her Doctorate in Osteopathic Manual Practice. |

In addition to her prior experience in a Scarborough multi-disciplinary health clinic, Pearl also has a solid background in Hotel Management and over 6 years of work experience in the food and beverage industry. Pearl joined the impressive Adelaide Health Clinic staff in December 2006 and is delighted to have the opportunity of working alongside some of the best health care practitioners she has known.
